Having a “Good Fit” between patient and doctor is critical for success!
PsychSynergy has a 3-Layer Process to see if we will be a “Good Fit” for you:
1. As a potential patient, you have read the entire website including the office policies and you understand and agree with said policies.
2. As a potential patient, you have completed the Pre-Screening Questionnaire form in its entirety with accurate responses.
3. As a potential patient, you attend your initial consultation for an in-person assessment.

PsychSynergy Practices is a small, solo, outpatient private practice. Thus, there is a limited number of new patients accepted based on the current resources, time and treatment strategies available.
All prospective new patients are REQUIRED to complete this questionnaire in order to learn more about you and to see if PsychSynergy Practices would be a good fit in assisting you with your treatment goals.
Due to call volume and office availability, we are only able to respond to new patient requests submitted through the screening form.
We appreciate the urgency of your need to be seen. In order to get back to as many people as possible, we ask that you please refrain from calling the office and instead fill out the form to request care at our center. Please answer all questions honestly.
After submission, you will be contacted within 24-48 hours to either:
- Schedule an initial consultation with psychiatrist Dr. Candace Giles; OR
- Be provided with referrals for other options to better address your treatment goals.
